La enfermedad renal crónica supone un reto para la Sociedad y una amenaza a la sostenibilidad del sistema sanitario debido a su creciente prevalencia, su repercusión sobre la calidad de vida y mortalidad de las personas y su alto coste económico. La prevención y detección precoz de la enfermedad renal crónica permite prevenir, frenar y revertir su progresión, permitiendo la mejora del bienestar de la población y ahorro de costes derivados del tratamiento de las complicaciones. Sin embargo, los mecanismos de detección precoz actuales consistentes en la evaluación de biomarcadores durante la visita médica del paciente individual no son eficientes y precisan una optimización. Los avances en los últimos años en los sistemas de información y el análisis masivo de datos facilitan la introducción abordajes novedosos de problemas de salud a nivel poblacional, hasta ahora inexplorados. Nuestra invención consiste en un software y algoritmos en esta línea, que, alimentados con datos poblacionales de biomarcadores de función renal, permiten la búsqueda automatizada de personas en riesgo o con daño renal y emisión de recomendaciones personalizadas para su cuidado. Este nuevo enfoque supera las dificultades de los métodos clásicos de detección de enfermedad renal y mejora la eficacia y eficiencia. Este software ha sido aplicado en práctica clínica, siendo la base del cuadro de mandos de la iniciativa “Observatorio de Salud Renal de Córdoba” desarrollada en el Hospital Universitario Reina Sofía. Esta iniciativa permite la transformación de un modelo diagnóstico con actitud reactiva a una innovación proactiva para la detección y solución rápida de los problemas. Utilizando la información derivada del software se ha realizado una experiencia piloto, donde ha demostrado que su uso en acción coordinada de Nefrología con Atención Primaria tiene un impacto disruptivo beneficioso para los usuarios del sistema sanitario, médicos de Atención Primaria, Servicio de Nefrología y Servicio Andaluz de Salud. La siguiente obra "La siguiente obra "Sistema para la identificación automática de personas con daño renal a partir de fuentes de datos clínicos." ha sido registrada como obra de tipo Software, bajo el expediente FIBICO-20020 por la Oficina de Transferencia de Tecnología del Sistema Sanitario Público Andaluz, (OTT-SSPA).
